What's the most common reason transformation projects fail?

Starting from the tool instead of the problem. When the goal is 'implement a CRM' instead of 'raise lead follow-up from 40% to 80%', the project has no success criterion — and in reality never reaches one.

How do we deal with team resistance to change?

Involve the people who will use the system in its design early, make a first small win visible within two weeks, and switch the old system off the day the new one goes live so no parallel state forms.

What should we do if the project stalls midway?

Use data to tell whether the problem is scope or people, then cut the scope in half and build one win; and if it must stop, stop explicitly and with the lesson written down — a project continued out of embarrassment burns the team's trust.
